It should be like topic of webpage
You can target your keyword in starting or middle of page title.

Yes it should start with your keyword, but remember:

1. If you have too much keywords which can't cover in the title then select the most important keywords which can be fit in the 65 character limit of the title.
2. Always create the title descriptive, like a sentence that covers your keyword(s).
