Does frequent changes to a website like changing the page extension, internal links, external links, content, design, etc. affect the search engine rankings?

I dont think so that some-kind of changes can affect on its PR.

Of course, frequently changes in website content, link or any other thing will affect the search engine ranking because the changes in the site will be estimated the quality of the site and on the basis it, the search engine ranking will be decided.
