How to De-index Pages from Google Permanently ?

i want to de-index the pages permanently ..using webmaster removal tool it will be only temporary removal

Total 623 Webpages Index in Google but today i checked index pages i got only 538 Page Index in Google why deindex my page in google. i am checked Some.....
